KP Workplace of the Future Video Contest 2014
Rules
- Each video should address the theme and not exceed 100 seconds in length (not including credits).
- Videos must be submitted by September 15, 2014 11:59pm (PDT)
- By Submitting a video you grant to Kaiser Foundation Hospital, Kaiser Foundation Health Plan, Inc., and The Associated Permanente Medical Groups, which are commonly and collectively known as the Kaiser Permanente Medical Care Program ("Kaiser"), and their personnel and contractors, all rights, title and interest, including copyright, in each and every original Work that you have submitted to Kaiser Permanente in connection with a contest or other promotional event or product. Please read , sign and submit the COPYRIGHT to KP RELEASE FORM
- Everyone who appears in your film, no matter how briefly, must sign an Actor Release Form so we may show your work inside or outside of Kaiser Permanente.
Copies of all your Release Forms (English, Chinese,
Spanish, Mid-Atlantic States,
Northwest ) must be submitted along with your film.
- Please refer to the Kaiser Permanente Principles of Responsibility to inform the content of your video. In particular pay attention to the sections regarding the use of KP devices, treatment of people and personal health information.
- Avoid violating copyright laws or using brand names or logos other than Kaiser Permanente's logo in your video. You agree to indemnify Kaiser Permanente for any claim, demand, judgment, or other allegation arising from your possible violation of someone's trademark, copyright, or other legally protected interest in any way in your video.
- The Kaiser Permanente logo cannot be manipulated or used in any way that is not approved in brand guidelines.
- Your video may not contain any endorsement, solicitation, commercial, or similar content advocating for or against candidates for office, political issues, services, or products.
- Your video must be original content for this contest. Do not submit a video that has been submitted elsewhere.
- The video must be submitted through the defined submittal process and comply with website terms. Submissions provided any other way will not be accepted.
- All contest entries become the property of Kaiser Permanente. This video cannot be publicly posted on the internet without the permission of Kaiser Permanente.
