Enter the Contest
Deadline to enter is September 15th, 2014 11:59pm (PDT)
Submitting an entry is a multi-step process:
- Ensure your video complies with the rules of the contest and that you have all required forms endorsed to avoid disqualification.
- Release forms for anyone in the video or if someone created music for it.
(English, Chinese, Spanish, Mid Atlantic State , Northwest ) - All entries require a Copyright form
- Fill out and submit the entry form
- Transfer your video along with the scanned copyright and release forms to us by using Kaiser's Secure File Transfer website -- https://sft.kp.org
Do not directly email the video file -- it will not work. If you are not able to scan your forms you can fax them to 510-625-7254 (but beware, it might affect your innovation score.)
Kaiser's Secure File Transfer Instructions
- Click on the https://sft.kp.org link or enter it into your browser's url address field.
- Use your Kaiser email address and your Single Sign-On password to login after the page loads.
- Click on the "Send File" tab on the top left of the screen.
- Input the following into the fields:
To = national-wfpd@kp.org
Subject = Video Contest
- Select the "Choose File" button and then identify the file to be transferred. You will need to repeat this process until all the required files are attached. (video file, all release forms, & copyright form)
- Type us a message in the text box. Give us the title of your movie and anything else you want to say.
- Press the "Send" button.
